ADMISSION REQUIREMENTS

 

(For New Students)

BASIC EDUCATION DEPARTMENT

1. GSO Pre-admission Completion Form
 
2. Medical Clearance from MDSO
→ Kindergarten to Grade 6 Applicants
Please see the MDSO staff at SAC Grade School for medical assessment
 
→ Grade 7 to Grade 10 Applicants 
Please see the MDSO Staff at SAC Main Campus for medical assessment)
 
→ Grade 11 Applicants  (Submit the results of the following Medical Examinations to MDSO at SAC Main Campus)
♦ Complete Blood Count (CBC)
♦ Urinalysis
♦ Fecalysis
♦ Chest X-ray PA View
 
3. Academic Requirements
* Kindergarten
♦ PSA Birth Certificate
♦ Baptismal Certificate
♦ 2x2 ID Picture
* Transferee to Grade 1-6
♦ Report Card (Original)
♦ Certificate of Good Moral Character (Original)
♦ PSA Birth Certificate (photocopy)
♦ 2x2 ID Picture (in collared shirt with blue background)
 
Note: Submit these documents in a brown long envelop to the department secretary of SAC Grade School
 
For Junior High School New Students
Admission Requirements
♦ Original Report Card
♦ Original Certificate of Good Moral Character
♦ Photocopy of PSA Birth Certificate
♦ 1 pc 2x2 ID Picture (in collared shirt with blue background)
 
ESC Requirements
♦ ESC Form 1 and 2 (ESC Application Form)
♦ Photocopy of Report Card
♦ Photocopy of PSA Birth Certificate
♦ Original and Photocopy of ESC Certificate (Transferees from other ESC Participating private high school)
♦ 2 pcs 2x2 ID Picture (in collared shirt with blue background)
Note: Submit these documents in two (2) separate brown long envelops to the Institutional Student Programs and Services  Office (ISPSO)
 
For Senior High School New Students
Admission Requirements
♦ Original Report Card
♦ Original Certificate of Good Moral Character
♦ Photocopy of PSA Birth Certificate
♦ Photocopy of Certificate of Completion
♦ 1 pc 2x2 ID Picture (in collared shirt with blue background)
SHS Voucher Requirements
♦ Original and Photocopy of ESC Certificate (For Students from ESC Participating private high school) or QVR Certificate (For Students from Non-ESC private high school)
♦ Photocopy of Report Card
♦ Photocopy PSA Birth Certificate
♦ 1 pc 2x2 ID Picture (in collared shirt with blue background)
Note: Submit these documents in two (2) separate brown long envelops to the Institutional Student Programs and Services  Office (ISPSO)
 

Keep a photocopy of all your academic requirements before submitting the original copy during admission since they cannot be borrowed once submitted.
