New Pupils - Submit your Entrance Credentials and Photocopy of receipt of down payment (PhP 3000.00) to SAC Grade School Secretary
 
Continuing Pupils - Submit your Original Report Card and Photocopy of receipt of down payment (PhP 3000.00) to SAC Grade School Secretary
 

High School

New Students - Submit your Entrance Credentials and Photocopy of receipt of down payment (PhP 3000.00) to ISPSO
 
Continuing students - Submit your Original Report Card and Photocopy of receipt of down payment (PhP 3000.00) to the Registrar's Office

NOTE: New students/pupils should undergo the pre-admission and admission process before they apply for registration.

Pre-admission of New Students

Pre-admission of pupils/students for AY 2024-2025 is on-going (except for BS Nursing)
 
Please follow the Pre-admission procedure below.
 
1. Please see the Student Development and Welfare Office (SDWO) to apply for an Intake Examination. We accept walk in applicants. Please come before 8:00 AM and prepare PhP 100 - 200 pesos for the Testing Fee.
 
2. Secure a laboratory request form from SAC Clinic. Submit all the required medical results to MDSO. (MDSO Contact Number:  09264889761 look for Miss Andrea)

2. Medical Clearance from MDSO
→ Kindergarten to Grade 6 Applicants
Please see the MDSO staff at SAC Grade School for medical assessment
 
→ Grade 7 to Grade 10 Applicants 
Please see the MDSO Staff at SAC Main Campus for medical assessment)
 
→ Grade 11 Applicants  (Submit the results of the following Medical Examinations to MDSO at SAC Main Campus)
♦ Complete Blood Count (CBC)
♦ Urinalysis
♦ Fecalysis
♦ Chest X-ray PA View
 
3. Academic Requirements
* Kindergarten
♦ PSA Birth Certificate
♦ Baptismal Certificate
♦ 2x2 ID Picture
* Transferee to Grade 1-6
♦ Report Card (Original)
♦ Certificate of Good Moral Character (Original)

For Junior High School New Students

ESC Requirements
♦ ESC Form 1 and 2 (ESC Application Form)
♦ Photocopy of Report Card
♦ Photocopy of PSA Birth Certificate
♦ Original and Photocopy of ESC Certificate (Transferees from other ESC Participating private high school)
♦ 2 pcs 2x2 ID Picture (in collared shirt with blue background)

For Senior High School New Students

SHS Voucher Requirements
♦ Original and Photocopy of ESC Certificate (For Students from ESC Participating private high school) or QVR Certificate (For Students from Non-ESC private high school)
♦ Photocopy of Report Card
♦ Photocopy PSA Birth Certificate
♦ 1 pc 2x2 ID Picture (in collared shirt with blue background)

Keep a photocopy of all your academic requirements before submitting the original copy during admission since they cannot be borrowed once submitted.
